Office Hours and Appointments
Our office is open Monday through Friday from 8:30am to 5:00 pm. Patients are seen on an appointment
basis. New patients are seen by physician referral only.
Every effort is made to see you at the time of your appointment. However, emergencies do arise and
we ask your patience when this happens. We will attempt to contact you whenever we have enough
advance notice to do so.
Occasionally, it may be necessary for us to change your appointment. Under these circumstances,
we will call you promptly and reschedule your appointment. Should you find it necessary to cancel or
change your appointment, please try to notify our receptionist 24 hours in advance.
Before you are seen by one of our doctors, you will be asked to complete a health questionnaire.
These questions are all necessary because they supply us with information essential to your evaluation.
Should you have problems with the questionnaire, we will be most happy to help you. If questions are
not understood or you don't know how to answer, simply put a question mark in front and the doctor
will discuss it with you.
At the first visit and subsequent visits with the doctor, please bring with you any medications
you are presently taking, as well as others that you have on hand. This will allow us to update
your medical records and avoid the expense of duplicating medications or running the risk of
prescribing medications that potentiate or counteract others that you have or may be given by
another physician.
Also at the time of your first visit, you are asked to bring all insurance information with you.
Telephone Calls
Should you encounter an emergency, please call our office at 355-0070. If it is after our regular
office hours, the answering service will refer your call to the physician on call.
We have instructed our receptionist not to interrupt us when we are seeing patients. Please leave
a number with the receptionist where you can be reached during the day and we will return your call
after morning or afternoon office hours. Emergency calls will be handled as quickly as possible.
If you are expecting a return call from one of us, please keep your telephone line open.
Prescription refills are issued during our usual office hours. For a routine prescription refill,
please contact your pharmacy and, if necessary, the pharmacist will call our office for a verbal
refill approval. Please do not call for routine refills after hours or on weekends and holidays
as records needed are not readily available.
Waiting Room
As you enter our office, please sign in at the receptionist's desk and be seated. We ask that
you keep cell phone calls to a minumum while in the waiting room as a courtesy to other patients
who are waiting. We also ask that you do not bring food or drinks into the waiting room.
Fees
Our fees for professional services are consistent with those charged in other offices in the
community. We will always be glad to discuss fees with you. Fees for an initial evaluation will
be higher than for a routine follow-up visit.
Billing and Insurance
Billing costs are constantly rising. In an effort to keep all medical costs down, we ask that you
pay for visits and treatments at the time the service is rendered. If for any reason this cannot
be done, please discuss any problems with the receptionist. We will work out a payment schedule
with you.
All statements are mailed out monthly. The statement you receive is a direct copy of the accounts
receivable card we keep for you. We ask that you make monthly payments on your account. Accounts
not showing payments after 60 days will be considered delinquent and collection efforts will be
started.
When you leave the doctor's office or examination room, you will be given a charge ticket. Please
take this to the receptionist on your way out.
The doctor's services are to the patient and payment of his charges is the patient's responsibility.
The doctor has no direct relationship with the insurance company. Collection of insurance benefits is
a matter between the patient and the insurance company but we will assist you in reporting claims to
your insurance company.
